Outreach Programs (MOT- TOP)
Search for Change’s Outreach Programs deliver intensive care management services to individuals transitioning from state-operated inpatient psychiatric facilities back into the community. Our multidisciplinary teams—comprised of an LMSW Program Manager, a Licensed Practical Nurse (LPN) Care Manager, and a Peer Specialist—provide personalized support to help each participant achieve their recovery and wellness goals.
Each individual receives a comprehensive support plan tailored to their unique needs and strengths. Our teams work collaboratively with participants to develop the skills necessary to live independently, maintain stability, and reduce the likelihood of future hospitalizations.
Safe Options Support Team (SOS)
The Safe Options Support (SOS) Team is an innovative initiative funded by the New York State Office of Mental Health. This program focuses on delivering compassionate, community-based care to individuals experiencing unsheltered homelessness. Our team prioritizes connecting individuals with safe, stable housing while addressing their mental health and wellness needs through ongoing engagement and support.
Home and Community-Based Services (HCBS)
Home and Community Based Services (HCBS) allow Medicaid beneficiaries with behavioral or mental health conditions or substance abuse disorders to receive services in their home or in their community

Community Oriented Recovery & Empowerment (CORE) Services allow Medicaid beneficiaries with behavioral or mental health conditions or substance use disorders to receive services in their home or in their community.
