Home and Community Based Services (HCBS) allow Medicaid beneficiaries with behavioral or mental health conditions or substance abuse disorders to receive services in their home or in their community.

- Create a Personalized Plan
Eligibility: Home + Community Based Services (HCBS) are available to Health and Recovery Plan eligible individuals. This includes those who are 21 years or older and are insured by Medicaid managed care plan.
