Search for Change takes a leading role within the community, providing services that enable people in need to live a fulfilled and empowered life. From our Home and Community Based Services (HCBS) to our Outreach Programs, we are at the forefront of community support when people need it most.

Home and Community-Based Services 

Search for Change is pleased to announce that we are now offering Home and Community Based Services.

Home and Community Based  Services (HCBS) include a package of services available to many Medicaid recipients in New York State. Recipients enrolled in select Medicaid Managed Care plans (known as Health and Recovery Plans or HARPs) may access HCBS if deemed eligible through a specialized assessment administered by a Health Home Care Manager.

Outreach Programs 

Search for Change outreach programs provide intensive care management services to individuals transitioning from an inpatient state-operated hospital setting. Dedicated and professional teams consisting of a LMSW Program Manager, a Licensed Practical Nurse (LPN) Care Manager and a peer specialist work with individuals to assist them in reaching their goals.

Our outreach programs have comprehensive support plans, which identify and deliver targeted goals for individuals to be discharged into the community.  Throughout the program, we assist in the development of the skills needed for the individual to maintain independence and remain out of the hospital.

Within all our community support services, Search for Change staff provides the utmost empathy, professional dedication, and time in assisting individuals with daily living tasks as well as advocacy services to coordinate treatment, rehabilitation, benefits, and general community integration.
