Mobile Outreach Team

The Mobile Outreach Team (MOT) provides intensive care management services to individuals with significant behavioral health issues throughout Westchester County. This team is comprised of two full-time case managers and a Licensed Practical Nurse (LPN), and it is equipped to manage a full array of rehabilitative services (including medication management and administration) to individuals who require more intensive care management services than are generally available through other providers.

The MOT program was established to facilitate the successful transition of patients from state-operated psychiatric facilities (e.g., Rockland Psychiatric Center) to various community-based settings. This program aids recipients in developing the skills and resources essential to the achievement of lasting stability and self-sufficiency in the least restrictive residential accommodations.

Transitional Outreach Program

The Transitional Outreach Program (TOP) provides intensive care-management services to individuals transitioning from an inpatient state-operated hospital setting. The team is comprised of an LMSW Program Manager, a Licensed Practical Nurse (LPN) and a Peer Specialist.

The Transitional Outreach Program provides in-reach services to develop support plans and identify goals before individuals are discharged into the community. Similar to the MOT program, individuals are assisted in the development of the skills needed to maintain stability and remain out of the hospital.
