A New Team-Based Approach Offering Comprehensive Community Care to Individuals Experiencing Unsheltered Homelessness.

Search for Change is thrilled to present The Safe Options Support (SOS) Critical Team Intervention (CTI) Program. This innovative program, funded through the New York State Office of Mental Health, is dedicated to delivering comprehensive community care to those most in need, particularly individuals experiencing unsheltered homelessness, with a primary emphasis on facilitating secure housing solutions.
The team is comprised of a Licensed Care Coordinator, Care Coordinators, CASAC, and Peer Specialists. These professionals offer intensive care coordination and support to individuals facing homelessness, many of whom are also navigating behavioral health challenges.
Through strategic partnerships with emergency shelters, outreach providers, and supportive housing programs, our SOS team plays a pivotal role in guiding individuals through the transition from homelessness to the stability of secure housing. Together, we are committed to empowering individuals, fostering resilience, and creating pathways to brighter futures.

We are pleased to add the  Safe Options Support (SOS): Critical Time Intervention (CTI) Program to our continuum of care. The SOS: CTI Program is an innovative approach to serving individuals experiencing unsheltered homelessness, many of whom require intensive outreach and engagement services in order to secure stable housing and other resources essential to their improved health and wellbeing.

This program will utilize a Critical Time Intervention (CTI) model, an evidence-based intervention proven to promote favorable outcomes among individuals experiencing transitions from institutional settings to community-based care. It is also applicable to individuals who require a comprehensive array of support services in order to achieve successful transitions from homelessness to housing. It is expected to promote improved engagement in primary, preventive, and behavioral healthcare services.

Services provided include, but are not limited to:

  • Targeted and assertive outreach to communities, local outreach providers, and other entities who regularly interact with unsheltered homeless persons (e.g., police departments, hospitals, family members, etc.) to identify individuals who would benefit from enrollment in the SOS: CTI Program.
  • Engagement of unsheltered homeless persons via evidenced-based practices such as CTI, Harm Reduction, Housing First, Trauma-Informed Care, and others proven to promote desired outcomes among individuals with intensive service needs and associated risk factors.
  • Delivery of a diverse array of social support and systems navigation services:
    • Assistance in the acquisition of basic necessities (e.g., food, shelter, transportation, etc.)
    • Assessment of recipients’ health and social welfare needs
    • Referrals to natural and professional support networks
    • And many more!

Referrals may be made by any individual or organization seeking services on behalf of unsheltered homeless individuals. These include hospitals, members of law enforcement, healthcare professionals, family members, and Departments of Social Services, among others.
