Community Oriented Recovery & Empowerment Services (CORE)

At Search for Change, our goal is to partner with you on improved health, wellness and recovery.

Community Oriented Recovery & Empowerment (CORE) Services allow Medicaid beneficiaries with behavioral or mental health conditions or substance use disorders to receive services in their home or in their community.

Family Support and Training:

  • Enhances Relationships
  • Engages the Family in the Recovery Process

Psychosocial Rehabilitation:

  • Skill Building for Settings Where You Live, Learn, Work, Socialize
  • Psychoeducational Interventions
  • Employment Support

Eligibility: Community Oriented Recovery & Empowerment (CORE) Services are available to Health & Recovery Plan eligible individuals. These individuals include those 21 years or older who are insured by a Medicaid managed care plan.
