Outreach Programs
Search for Change outreach programs provide intensive care management services to individuals transitioning from an inpatient state-operated hospital setting. Dedicated and professional teams consisting of a LMSW Program Manager, a Licensed Practical Nurse (LPN) Care Manager and a peer specialist work with individuals to assist them in reaching their goals.
Our outreach programs have comprehensive support plans, which identify and deliver targeted goals for individuals to be discharged into the community. Throughout the program, we assist in the development of the skills needed for the individual to maintain independence and remain out of the hospital.
Safe Options Support Team
This innovative program, funded through the New York State Office of Mental Health, is dedicated to delivering comprehensive community care to those most in need, particularly individuals experiencing unsheltered homelessness, with a primary emphasis on facilitating secure housing solutions.
